Aussie Hoops Terms and Conditions upon Registration of a Participant
To be agreed to by person listed as Parent / Guardian 1 as the participant is under 18 years of age.
I am the parent or guardian of the applicant. I authorise and consent to the applicant undertaking the BA Activities. In consideration of the applicant’s participation in the Program, or membership with BA and any other BA Organisation being accepted, I expressly agree to be responsible for the applicant’s behaviour and agree to accept in my capacity as parent or guardian, the terms set out in this membership declaration. In addition, I agree that the applicant and I are bound by, and shall comply with, the BA Constitution and any by-laws and policies made under it.
I have read, understood, acknowledge and agree to the above including the exclusion of implied terms, warning, assumption of risk, release and indemnity.
